Dr. Mel Lobo, Hypertension Specialist Ambulatory Blood Pressure Monitoring

Outpatients

Dr Lobo undertakes outpatient consultations for hypertension and cardiovascular disease in both the NHS and private practice. He runs the Hypertension and Cardiovascular Risk Reduction service at Bart's and the London NHS Trust and sees in excess of 2000 hypertension-related consultations per year.

Inpatients

Patients with difficult to control blood pressure sometimes require hospital admission to further assess their hypertension and safely reduce blood pressure using either intravenous infusions of anti-hypertensive therapy or tailored oral therapies. Dr Lobo has facilities for admitting both NHS and private patients for such reasons.

Over the years Dr Lobo has developed safe and highly effective protocols for attaining blood pressure control in even the most difficult of cases thus reducing the risk of these patients experiencing debilitating cardiovascular events such as stroke or heart attack. Many of these cases have been written up or presented at local/national meetings to help other doctors to learn about difficult hypertension.

Ambulatory Blood Pressure Monitoring

24 hour ambulatory blood pressure monitoring (ABPM) is the gold standard investigation to determine whether or not a patient truly has hypertension, how serious it is and also how good control of blood pressure is in treated individuals. It is unquestionably the single most reliable guide to prognosis that is easily available and is a non-invasive test which is easy to organise and perform without causing inconvenience to patients.

Dr Lobo undertakes 24 hour ABPM on nearly all patients at some stage to assist in the diagnosis and management of their hypertension. This is done using state of the art equipments from Spacelabs which is regularly sent for maintenance and recalibration to the manufacturer. He also reports all 24 hour ABPMs undertaken in Bart's and The London NHS Trust (up to 20 per week) to assist Consultant colleagues from other specialties in the management of their patients with hypertension.

Home Blood Pressure Monitoring

An increasing body of evidence is accumulating which supports the use of home BP monitoring as a guide to treatment and prognosis of patients. In fact the American Heart Association Council for High Blood Pressure has called for US insurers to reimburse patients for the cost of purchasing monitors and recommended in a recent position statement that ALL hypertensive patients should make use of home monitoring to enable better management of hypertension

All of Dr Lobo's patients are provided with information on how to purchase a reliable monitor for home use and given instruction in how best to measure and document their blood pressures whilst in the community.
